This document outlines how to configure and work with the Sample Form features. To configure Sample Forms, you should first create Custom Fields.


NOTE – Custom Fields can only be managed by a company administrator.

Manage Sample Form Fields


To configure the sample form:

  1. From the ADMIN section, select SETTINGS from the left navigation menu.



  2. Select the SAMPLE FORM tab along the top of the page.

    The system default sample fields are listed.





Add Fields to Sample Form


To add a field to the sample form:

  1. Click on [ADD FIELD]. A new field displays at the bottom of the list.



  2. Select a field from the drop-down list.

  3. Specify the Validation for the field.

    • Optional – The field is not required when adding a new sample.
    • Required – The field is required when adding a new sample.


  4. Specify the Width that the field displays on the form.

    • 25% – displays 1/4 of the width of the form.
    • 33% – displays 1/3 of the width of the form.
    • 50% – displays 1/2 of the width of the form.
    • 66% – displays 2/3 of the width of the form.
    • 75% – displays 3/4 of the width of the form.
    • 100% – displays the entire width of the form.



  5. To change the order the field displays on the form, click on [:] and drag the field to the desired position.



  6. Click on [SUBMIT].





Add Trigger Logic to Fields

A Trigger is a logical condition that must be satisfied for the field to be presented on the form. When you add a trigger you specify the trigger condition and answers. Multiple triggers are allowed on a field. When multiple triggers are added they form an "and" statement, whereby the parameters from all triggers in the set need to be true for the trigger to apply.

To add a trigger to a field:

  1. Add fields following the Add Fields to Sample Form instructions above.



  2. Click on ADD TRIGGER.



  3. Select the Field to base the logic on.

    NOTE – Fields selected on a trigger can be sourced from the Sample Form or Project Request Form.



  4. Specify the Condition.

    • Includes Any True if matching at least one of the selected answers.
    • Includes None True if anything other than "matching at least one of the selected answers".



  5. Select the Answers the logic is based on.




  6. Click on [SUBMIT].





Delete Fields from Form

Fields that are no longer needed can be removed. Any data previously entered on existing projects is retained. Deleted fields can be added back to forms, as needed. 


To delete a field from the form:

  1. Click on [DELETE].


    The field is removed from the form.


  2. Click on [SUBMIT].
