Pages group questions together for presenting to a participant. A questionnaire must have at least one page and is presented to a participant one page at a time. Pages can contain one or more questions. You can add new pages and add questions to pages to build your questionnaire. Questions can also be moved from page to page, as desired.


Use the Page Tools to modify page settings, insert or remove page breaks, copy entire pages, save a page template to a library, or delete a page and all questions on the page. Click on Add New Page to add more pages to the questionnaire and reorder pages, as desired.

Collapse/Expand Question Display Mode

Questions can be collapsed to display a minimized view of the questionnaire layout, leaving only the first line of question text displayed. When expanded, the full question text and responses display. Questions can be modified, by clicking on a question to open the editor, in either display mode.


To collapse/expand questions:

  1. From the questionnaire builder top bar, click COLLAPSE/EXPAND ALL PAGES. Applies the display mode to all questions on all pages.



  2. From a page, click COLLAPSE/EXPAND ALL. Applies the display mode to all questions on the page.



  3. From a question, click the triangle icon to collapse/expand the individual question.




Add a New Page

A questionnaire may contain multiple pages.


To add a new page: 

  1. Below the questionnaire page, click on + Add New Page.



    A new page is added below.




Modify Page Settings

In page settings you can specify question randomization and add a trigger, a logical condition that must be satisfied for the page to be presented. Multiple triggers are allowed.

To modify page settings: 

  1.  From a questionnaire page, click on the Page Settings icon.



  2. Select the Question Randomization from the drop-down menu or continue with adding a trigger, as desired. See   Configure Questionnaire Logic: Add or Delete a Trigger.




Reorder Pages in a Questionnaire

If you are not satisfied with the order of pages in the questionnaire, you can move them around and reorder them.

NOTE – When question trigger types exist on the page, a warning will display to check the question selection on trigger logic within the page.


To reorder pages in a questionnaire:

  1. From the bottom of the page, locate the up/down arrows in the lower right corner. Click to move the page up or down accordingly.



  2. From the bottom of the page, locate the [Page #] drop-down menu in the lower right corner. Click and select a new page number from the drop-down. The page is moved to the selected page number location within the questionnaire, the page that previously had the number is now below it.




Page Breaks

Page breaks can be inserted between two questions on a page. You can also break all questions on a page so that each question is on a page of its own. Page breaks can be deleted to combine two adjacent pages into one page. Inserting a page break from the first question on a page will add a new empty page above it.


NOTE – When inserting page breaks the existing page settings are copied to each new page that questions are broken out to. When deleting page breaks the existing page settings from the bottom page are discarded, only the page settings from the top page are retained.


To insert or delete a page break: 

  1.  Select a question on a page, click on the Page Break icon.

    The question is split to a new page.


  2. From a questionnaire page, click on the Break All icon.



    Click [OK] to confirm.


    All questions and page elements are split out on to individual pages.


  3. From a questionnaire page, click on the Delete Page Break icon.


    The questions on the page are combined with the page above it.




Duplicate a Page

Copy an existing page to save time in configuring similar or a second set of repeated questions. Move it to the desired location and configure it, as desired.

NOTE – Question triggers on the new duplicate page will automatically update when the triggering question is within the same page. If the triggering question is from another page, the question trigger is not changed.


To duplicate a new page: 

  1.  From a questionnaire page, click on the Duplicate Page icon.


    A duplicate page of questions is added below the existing page. You can modify the question configurations and reorder the page, as needed.




Save a Page Template

Saving a page to a library creates a template that you can access to reuse questions for other questionnaires.

To save a page template:

  1. From a questionnaire page, click on the Save to Library icon.



  2. Select the library from the drop-down list where you wish to save the page template.



  3. Select the folder or check Create New Folder and add one.



  4. Complete the Template Name, then click on [SUBMIT].


    The page template is saved to your designated library.



Delete a Page

You can delete pages you no longer need. Deleting the page will also delete all questions on that page.


NOTE – If data has been collected, deleting the question will also delete its results in analyses. To preserve the question's existing data, instead of deleting it, add a trigger that will effectively cause the question to never be asked. For example:
  • a question based trigger, from a single selection question, using the the answer condition "is exactly this", with all the answers checked.


To delete a page:

  1. From a questionnaire page, click on the Delete Page icon.



    Click on [OK] to confirm.
