Use a no samples questionnaire when no samples are being evaluated, such as for concept, visual, demographic or usage and attitude questionnaires. A concept questionnaire, for example, may be served before samples.
To create a no samples questionnaire:
From a project, locate the test and click on the Surveys tile.
From the Questionnaires step, click on [CREATE QUESTIONNAIRE].
Complete the Title field.
- Select No Samples from the Questionnaire Type drop-down menu.
- Sample Entry Mode is Skip by default. When Verify is selected, the sample code entry screen is displayed and a "99" code needs to be entered to start the no samples questionnaire. When Confirm is selected, the sample code confirmation screen with the code "99" displays.
SEE ALSO – Non-Sample Entry Mode for further details. - Sample Display is Code by default. This displays the code "99" on the sample code confirmation screen as described in step 5 above. When Name is selected, the questionnaire Title, as entered in step 3, will display on the sample code confirmation screen.

The questionnaire is added. - Click on [SAVE & EXIT] to quit or click on [NEXT] to continue with the Project Wizard.
This completes the steps for adding a "no samples" questionnaire. Continue working with a questionnaire by adding questions, create another questionnaire or go to the Design Block.
